The following case is an example of an uncommon underlying condition, papillary fibroelastoma, which presented with a stroke. It highlights the importance of a full relevant medical workup, an awareness of uncommon etiologies in the workup of any significant clinical events, and the need for collaboration between medical and surgical colleagues.
